There will be no shame for them

The June examination season at theMedical Institute is always a significant event in the life of Tula State
University.
The state final certification in General Medicine is conducted in two stages. First, there
is certification testing, and then an interdisciplinary interview, which includes three situational clinical tasks of an integrative nature in therapy,
surgery, obstetrics and gynecology.
An interdisciplinary interview is essentially an immersive exam. The graduate is immersed in his professional
environment. Natural excitement in such cases, a special atmosphere, attentive glances of teachers... And in such a serious environment, the student needs to
mobilize all his internal reserves, correctly diagnose and prescribe the appropriate treatment. And even if the patients in the exam are virtual, the
future doctor must solve his problems quickly, as if there was a living person in front of him who needed his help.
On June 19, we managed to slightly
touch this mystery.
Anastasiya Burminskaya has already sighed with relief. She passed the exam successfully. Two A's and a B - isn't
that a victory!
The girl came from the capital of our Motherland to study at the Tula State University Medical Institute. Yes,
the Muscovite considered this the best option for herself, so she entered purposefully.
In therapy, Anastasiya
"got" a patient who had just returned from a vacation in Spain. The man had a high temperature, a dry cough, a sore throat, and a cold. He was
taken by ambulance to the infectious diseases department of the hospital.
— COVID was diagnosed, — Anastasiya said. — I prescribed the patient genetically engineered drugs and human
interferon. Plus symptomatic therapy.
In surgery, Anastasiya had a patient with mesenteric lymphadenitis. The woman was brought in by ambulance with
suspected appendicitis. But diagnostic laparoscopy showed that the appendix was not inflamed, and the mesenteric lymph nodes were enlarged.
— I prescribed antibiotics to the
patient, — Anastasiya said. — And, of course, I refered her for a lymph node biopsy to rule out cancer or tuberculosis of the lymph nodes.
In obstetrics and gynecology, a
young woman was admitted to Anastasia at 36-37 weeks of pregnancy with bloody discharge from the genital tract.
Based on the ultrasound examination,
placenta previa was diagnosed. The woman was urgently refered for a cesarean section.
The baby will be born prematurely,
but, according to the future doctor, his life is not in danger.
Anastasiya plans to enter residency in the specialty "Anesthesiology and reanimatology". According to the
girl, she made this choice after two and a half years of working in an ambulance in the village of Kosaya Gora.
- Until the third year, it was very
difficult to study! - the student shared. - But we had a very friendly group,and we overcame all the difficulties together.
Anastasiya spoke especially warmly
about her teachers.
- Margarita Aleksandrovna Bondarenko, who taught us Latin, is a world-class person! She prepared us for
what we would have to face in life, - said the graduate. - Victoria Ilyinichna Chmilenko taught us the subject "Normal Physiology". She always said:
"When explaining a subject, I translate it from Russian to Russian for you."Olga Viktorovna Semencheva taught anatomy. She is strict, but fair. And very
experienced! In general, I am grateful to all my teachers! 
Tula resident Anastasiya Klyuyeva is not from a family of doctors, but she always wanted to connect her life with
medicine.
The student studied under a target agreement concluded with City Hospital No. 9 in Tula. She, like the previous
Anastasiya, is going to enter the residency at Tula State University in Anesthesiology and Reanimatology.
Both girls preferred, let's say, the
male specialization. It's no secret that the work of anesthesiologists and resuscitators is very, very difficult.
Anastasiya Klyueva also passed her
exams well.
Her first patient was a 56-year-old engineer with unstable angina pectoris. The man had a myocardial infarction.
Anastasiya prescribed him aspirin, fentanyl with droperidol and beta blockers.
- The patient will live! - she confidently declared.
The second case turned out to be
very serious. The patient had rectosigmoid colon cancer. Since the patient was admitted with peritonitis and intestinal obstruction, he was immediately
operated on.
He has chemotherapy and a long recovery ahead.
- And we hope for positive dynamics!
- Anastasiya emphasized.
The next patient is a young woman whose pregnancy was accompanied by preeclampsia (high blood pressure after the
20th week).
She was hospitalised in the pregnancy pathology department. There she was prescribed intravenous magnesium.
And if the clinical picture persists, the question of delivery will arise.
- Of course, I am deeply grateful to all my teachers! – said Anastasiya. - But I was especially impressed by those
who taught us in the junior courses. These were Margarita Aleksandrovna Bondarenko, Olga Viktorovna Semencheva, Yuliya Lvovna Venevtseva, Lyubov
Viktorovna Morozova. I would especially like to mention Yuliya Lvovna. She is a true professional in her field!
Georgiy Tskipuri had to provide therapy to a 68-year-old man with liver cirrhosis caused by hepatitis B and
alcohol abuse.
- We will fight for the man! - said the student. - I prescribed hepatoprotectors, a multivitamin complex and enzyme
preparations. I strongly recommended that he give up alcohol and stick to a diet. And if this man follows my recommendations, he will be able to return to normal
life.
As for surgery, this was a rather complicated case - type 1 diabetes (insulin-dependent) with diabetic foot
syndrome along with a stomach ulcer.
- Metabolic and angiotropic therapy,dressings with water-soluble ointments - this is the range of prescriptions, -
said Georgy. - Amputation of the limb is out of the question for now.
In obstetrics and gynecology, the guy had to deal with endometrial cancer of the uterus.
- It was necessary to save the
woman! Therefore, extirpation of the uterus, and then chemotherapy, - explained the future doctor.
Georgiy has an agreement on targeted
training in residency in the "Psychiatry and Narcology". After completing it, he will have to work at the Tula Regional Narcological
Dispensary, located on Mosin Street.
- For me, the years of study are daily work and self-development, - said Georgiy. - Thank you to all our
teachers who led us along this noble path and gave us a piece of themselves,for their patience, faith in our strength and kind attitude towards us!

Abdulhalem Mustafa Muhammad
Abdulhalem from Iraq passed his exams with good and excellent marks. He really hopes to enter the residency program at Tula State University in the specialty
of "Obstetrics and Gynecology". In general, the guy has far-reaching plans. He wants to stay and work in Russia and glorify the Tula region with his
work.
- Years of study? Now, after passing the certification, all these long six years seem to me like one big day. And in
this big day there were so many wonderful moments and so many bright feelings!When I successfully passed the exams, when I was present at various operations,
be it pediatric surgery, thoracic, abdominal... - Mustafa admitted. - I am grateful to all our teachers who shared their knowledge, experience and their
big hearts with us!
This year, two state examination commissions worked.
The chairman of one of them is a
candidate of medical sciences, head of the admission department - leading surgeon of the department of the Federal State Budgetary Institution "Main
Military Clinical Hospital named after Academician N.N. Burdenko" of the Ministry of Defense of the Russian Federation Evgeniy Viktorovich Yudin.
- This is my second time at Tula State University. And I am increasingly convinced that the level of training of
students is improving every year, - he emphasized. - It is gratifying to see how dynamically the Medical Institute of Tula State University is developing.
The teachers here are real enthusiasts, people who know and love their work.They put their souls into their students. And when all these guys come to the
sick, the Medical Institute of Tula State University will not be ashamed of its students.
The candidate of medical sciences,
deputy chief physician for standardization and development, otolaryngologist of the “Medskaya na Yauza Hospital LLC” (Moscow) Elena Anatolyevna Dubtsova spoke for
the first time as the chairperson of the examination committee at Tula State University.
- I have only positive impressions!
- she shared. - Our future colleagues, and you can't call today's graduates anything else, demonstrated deep knowledge of clinical disciplines. It is
obvious that they love their chosen profession, want to improve in it, they are interested in modern methods of diagnosing and treating diseases. I would also
like to note the high stress resistance of your students. Many thanks to the teachers of the Medical Institute, who were able to instill a love for this
noble craft in students and developed clinical thinking in them! All this gave worthy results, which we have the opportunity to observe now. I can`t help but
mention the highest level of organization and conduct of the exam. This indicates the great and well-coordinated work of the management and the entire
team. I also learned about unique teaching aids for students developed by the faculty of the Medical Institute. I will take several copies with me to Moscow
to review.
